Qoncier Notice of Privacy Practices (HIPAA)

Qoncier, Inc. ("Qoncier," "we," "our," or "us") is committed to protecting your health information. This Notice of Privacy Practices describes how we may use and disclose your protected health information (PHI) and your rights under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
1. Our Responsibilities

We are required by law to protect the privacy and security of your PHI.

We will notify you in the event of a breach of your unsecured PHI.
2. How We May Use and Disclose PHI
We may use or share your PHI in the following ways:
For Treatment
To help coordinate or manage your care with healthcare providers, trainers, or caregivers you authorize.
For Payment
If applicable, to process payments for services provided through Qoncier.
For Healthcare Operations
To improve Qoncier's platform, ensure compliance, and support administrative functions.
As Required by Law
To comply with federal or state regulations, public health reporting, or legal proceedings.
With Your Authorization
For any other purpose not listed above, we will obtain your explicit permission before using or disclosing your PHI.
3. Your Rights Regarding PHI
You have the right to:
Access
Request a copy of your PHI in electronic or paper format.
Amend
Ask us to correct PHI you believe is inaccurate or incomplete.
Restrictions
Request limits on certain uses or disclosures.
Confidential Communications
Ask us to communicate with you by alternative means (e.g., secure email).
Accounting of Disclosures
Request a list of certain disclosures of your PHI.
Revoke Authorization
Withdraw your permission for us to use/disclose PHI at any time.
Requests can be made by contacting our Privacy Officer (see Section 7).
4. Your Choices
You may decide how much PHI to share with professionals or caregivers through Qoncier. Permissions can be granted, limited, or revoked directly within the app.
5. Data Security
We protect PHI through:
HIPAA-Compliant Hosting
AWS HIPAA BAA in place.
End-to-End Encryption
TLS in transit, AES-256 at rest.
Role-Based Access Controls
Strict permissions limiting data access by role.
Continuous Monitoring
Continuous monitoring and audit logging.
6. Changes to This Notice
We may update this Notice from time to time. Material changes will be shared through the app or by email before taking effect.
