QONCIER TERMS OF SERVICE

Section 1
Scope of Service
Qoncier is a consumer health organization and educational platform. It helps users capture health notes (such as meals, sleep, stress, movement, and symptoms), organize health records, translate medical reports into plain language, and view summaries of health-related trends over time.
Qoncier may also allow users to maintain household profiles, including profiles for family members and family pets, to track wellness information such as nutrition, activity, symptoms, medications, and other health-related notes.
Qoncier also enables users to optionally share selected information with healthcare or wellness professionals in order to support more informed conversations about health and wellness.

Qoncier is not a medical provider and does not provide medical advice, diagnosis, or treatment. Any insights, summaries, or AI-generated outputs are provided for informational and educational purposes only. Do not use Qoncier for medical emergencies. If you believe you are experiencing an emergency, contact local emergency services immediately.
Section 2
Eligibility & Accounts
You must be at least 18 years old (or the age of majority in your jurisdiction) to create an account, unless a parent or legal guardian creates and manages an account for a minor as part of a Family plan.
Family plans may allow users to create and manage profiles for:
  • children or dependents
  • other household members
  • family pets
Users are responsible for the accuracy of the information entered for any household profiles they manage.
Healthcare and wellness professionals may create professional accounts in order to connect with users who choose to share information with them through the platform.
You are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account.
Section 3
User Content & Inputs
You may input or upload information, including health notes, wellness information, veterinary information for pets, medical documents, and other materials ("User Content").
You retain all rights to your User Content.
You grant Qoncier a limited, non-exclusive license to process your User Content solely for the purpose of providing the services you request, including:

Plain Language Translation
Informational Outputs
Historical Trends
Qoncier does not claim ownership of your User Content. You are responsible for ensuring that any information you upload or enter into the platform is accurate and that you have the right to share such information.
Section 4
Sharing, Access Controls, and Contributions
Qoncier provides features that allow users to share selected information with individuals both within and outside their subscription plan. The following terms govern how sharing, access, and contributions work within the platform.
Care Circle
Qoncier allows users to share selected information with individuals outside their subscription plan ("Care Circle"), including healthcare professionals, caregivers, or other trusted persons. Care Circle access is distinct from profiles managed within a Family Plan, which are administered inside the subscription.
User Control
Users control who has access to their information, what information is shared, and whether others may contribute. Users may modify or revoke access at any time through their account settings.
Contributions (Add-Only)
Individuals authorized through the Care Circle may add information — such as notes, updates, or observations — to a user's profile if permitted by the user. Authorized contributors may not edit or delete existing user data. All contributions are subject to the user's ongoing control.
Attribution & Activity Records
Qoncier may record actions taken within the platform with attribution (including the identity of the acting party where available) and timestamps, in order to maintain transparency and data integrity. Users may view activity records related to their account through the platform.
Invite Lifecycle
Access to a user's information may be granted via invitation. Invitations may expire if not accepted within a designated period. Users may revoke access at any time, after which the authorized individual will no longer be able to view or contribute to the user's information.
Professional Access
Healthcare and wellness professionals granted access through the platform may only view information explicitly shared with them by the user. Professionals may add professional notes or observations if the user has enabled this permission. Professionals may not modify or delete existing user data.

In-App Notifications & Activity Tracking: Qoncier may generate in-app notifications related to sharing events, access changes, and contributions to support user awareness and platform transparency. By using the service, users acknowledge that activity tracking and in-app notifications are a standard part of the platform experience and are used to maintain system integrity and user control.

Important Disclaimers (No Medical Care)
Qoncier does not replace the judgment of licensed healthcare professionals or veterinary professionals.
Information provided by Qoncier — including AI-generated summaries, educational information, or wellness insights — is intended for informational and educational purposes only.
Medical Emergencies
Qoncier does not provide emergency services. If you believe you may be experiencing a medical emergency, call local emergency services immediately.
Medications & Treatments
Do not change medications, treatments, or care plans based on Qoncier outputs without consulting a licensed healthcare professional.
Pet Health
For pet health concerns, users should consult a licensed veterinarian.
Section 6
Privacy & Security
Qoncier uses administrative, technical, and physical safeguards designed to protect personal information and sensitive health-related data.
Details about how we collect, use, and protect information are described in our Privacy Policy, which is incorporated into these Terms by reference.
Qoncier may use HIPAA-eligible infrastructure and may maintain Business Associate Agreements (BAAs) with certain service providers. However, whether HIPAA applies to specific information may depend on how the services are configured and used.
Users control what information is shared with healthcare professionals through the platform.
Section 7
Subscriptions & Payments
Certain Qoncier features may require a paid subscription.
Subscription terms, including free trials, renewals, cancellations, and refunds, are described in our Subscription Terms & Refund Policy, which is incorporated into these Terms by reference.
Healthcare Professionals
Healthcare professionals may create a professional account at no cost to connect with users who share information with them. However, professionals who wish to use Qoncier to track their own personal health information must subscribe to an individual or family plan.
Third-Party Billing
If you subscribe through Apple App Store or Google Play, their billing and payment terms may also apply.
Section 8
Acceptable Use
You agree not to misuse the service, including but not limited to:
attempting to access another user's data without authorization
interfering with the security or integrity of the platform
reverse engineering or attempting to extract source code
using the service for unlawful, fraudulent, or harmful activities
Users must respect the privacy of others and may only share information with professionals or caregivers when they have the right to do so.
Healthcare professionals agree to access shared information only for legitimate professional purposes and in accordance with applicable laws and professional obligations.
Section 9
Limitation of Liability

To the maximum extent permitted by law, Qoncier will not be liable for indirect, incidental, special, consequential, or punitive damages, including loss of profits, data, or goodwill arising out of or related to your use of the service.
Qoncier provides informational tools and educational resources and does not guarantee health outcomes, medical accuracy, or diagnostic conclusions.
Section 10
Changes & Termination
Service Updates
We may update the service or these Terms from time to time. If changes are material, we will provide notice as required by law.
Suspension or Termination
We may suspend or terminate access to the service if a user violates these Terms or if such action is necessary to protect the security, integrity, or lawful operation of the platform.
User Discontinuation
Users may discontinue use of the service at any time.
